Ultra Despair Pussies reimagines the Danganronpa premise as an adult visual novel built with Ren'Py. Players assume the role of Makoto, one of thirteen freshmen confined within Hope's Peak Academy. Rather than investigating murders, the focus shifts entirely to seduction and sexual domination of fellow students. Monokuma presents new rules encouraging players to sexually convince their assigned targets and accumulate conquests.

Gameplay centers on freely exploring the academy, interacting with various characters, and engaging in optional mini-games. The title features multiple collectible elements including trading cards obtainable through in-game currency and achievements. Players can uncover secrets and unlock additional content as they progress through the limited five-day cycle in the current early-access version.

The game includes several female characters from the original series and introduces explicit sexual scenes as primary content. Early access currently offers approximately 40 minutes to one hour of gameplay with four explicit scenes and roughly 30,000 words of content. The developer plans future updates to extend the day limit and add additional scenes, with Patreon supporters receiving early access to new content alongside exclusive bonus scenes. The game supports English and multiple languages through machine translation.
